Little is known about how patients value the benefits and burdens of palliative care. In this study, focus groups with terminal cancer patients were used to explore their attitudes toward current palliative care and to elicit their preferences for future services. The study was conducted at three teaching hospitals in Canada. All subjects were aged 18 years or older with advanced cancer, and were enrolled in a prospective randomized trial. Subjects were assigned to either group A (n = 21) or group B (n = 11) and participated in a discussion group at a time of their choosing (e.g., palliative care unit, outpatient clinic, emergency department). Discussions were audiotaped and transcribed. Thematic content analysis was performed on a random sample of audiotapes to identify key themes. The analysis revealed several key themes: current palliative care services and the patient-empowering paradigm were viewed as two separate elements; patients' relationship with the health care team was inconsistent with the patient-empowering paradigm; the patient-empowering paradigm was perceived to be new, changing, and evolving. Several recommendations are offered, including the need for greater collaboration between palliative care and other clinicians, and the need to enhance communication skills of non-palliative care providers. The patient-empowering paradigm is a new and unique model that requires further development. Better understanding of the patient-empowering paradigm can lead to the inclusion of patients in the clinical decision-making process and help optimize the delivery of palliative care services.package org.junit;
